If you are a new patient, please click on the New Patient Information tab above. For scheduling, please review the options below and choose the visit type that makes the most sense for you.  We are currently allowing patients to request appointments.  Once requested, our staff will review and make sure we have the necessary information to complete the booking.  Currently, Dr. Cleveland is not accepting new patients. We are offering Initial Consultations with our other Integrative Health Provider, Valerie Fox, RDN, who works in collaboration with Dr. Cleveland.
