New Patient Information

Thank you for your interest in Docere Functional Health.  We have two Consultants to choose from.

Dr. Emily Cleveland, our Naturopathic Doctor (ND), maintains her license as a primary care physician in Oregon, but since the State of Tennessee does not recognize licensed NDs, she is only able to provide information and education in the form of a health consultation and will not diagnose, treat, or cure any diseases.

Valerie Fox RDN, our Integrated Health Consultant, is also a Registered Dietitian Nutritionist and specializes in kidneys, gut, autoimmune disease, CIRS and mold/lyme. Valerie, also, is only able to provide information and education in the form of a health consultation and will not diagnose, treat, or cure any diseases.

 

What makes our office unique in Tennessee is that we collaborate as a team with our physician assistant and nurse practitioner when there is a need for laboratory evaluation and/or medications.  We do recommend you maintain a licensed primary care provider to manage your overall health needs.

Dr. Emily Cleveland's Fee Schedule:

Initial Consult (60 min.) is $350

Second Consult or 1st Follow-up (45 min.) is $225

Thereafter, most folks select a Follow-up Consult (30 min.) for $175

Valerie Fox's Fee Schedule:

Initial Consult (60 min.) is $325

Second Consult or 1st Follow-up (45 min.) is $199

Thereafter, most folks select a Follow-up Consult (30 min.) for $165

We are not allowed to accept insurance for visits.
